No. My cousin is a nurse and she said it's so useless and it never goes according to plan and is a waste of time lol

I've heard from many people it's a waste of time & never goes according. I had one that I printed out offline & filled out but I ended up never giving it to the nurses because I forgot all about it. My labor & delivery went smoothly & all of the stuff I had written down they usually ask you anyway ( who you want in the room for the birth, breast or formula feeding, skin to skin directly after birth, when you want visitors, etc etc )

Yes i did and will with this baby its actually not useless if you have specific things you want done.. Ie delayed cord clamping no hep b and the list goes on. I give it to my doc before birth at an appt and keep copies for the hospital stay. You can include important things you want or DO NOT want. And while yes labor and delivery are unpredictable it helps to have a guideline so the nurses and staff know what you want. It worked well for us.

My hospital gave me a form. All I did was check boxes for what I did or didn't want. There's space for personal notes/requests also. Mine came in handy because I couldn't talk much. It's good if youre alone or don't have any advocates to speak for you in case something goes wrong.

I never did just told my husband what I wanted. Also told my OB and the nurse who was in the room with me. They wrote it down in my chart and on a whiteboard in the room. It was useless anyways because my daughter's heart rate almost stopped so I had to get an epidural for her although I didn't want one. The only thing that stayed the same was she was placed directly on my chest but had to be taken within a minute to make sure she was ok and delayed cord clamping. Everything else I wanted went out the window.

@mommyx6 yeah see I had told them what I wanted and they didnt follow through. They cut the cord when I asked to delay it. Cleaned him then gave him to me when I wanted him asap.. Just horrible. I have to deliver there again so I want them to follow through.

I printed out an empty form online and filled it out. I made sure all the different nurses saw it as well as my doctor of course

I typed mine up and gave to my nurse and one for my husband just so if anything were to happen he would know what to say and do because I'm sure he'd be lost and overwhelmed had anything gone wrong

@kaseylyn that's what I was looking to print one. Unfortunately my dr doesn't deliver my children. Idk our ucsf clinic is strange so its a random dr during labor
