@jasmine.moore7509 im pretty sure you can fill them out and turn them in and once you finally go out call whoever handles your leave and tell them you're out of work. I would definitely talk to your HR and find out because sometimes it can take a while to be processed depending on how many people are on leave thru out your company

I think it depends on your work place.. I can’t do mine until 30-35 days before my due date.

i was glad my doctor took me out of work my twins were born at 34 weeks if i would of kept working i would of had them way earlier

@jahmiliapayne, okay thank you im going to go ahead and send an email before I leave. I had an appointment today and I don’t go back till 7/27 but after that I start going every two weeks I’m like wow that snuck up on me

I did mine at 25 weeks but my doctor took me out of work at 26 weeks

My employer actually never had me fill anything out. They just approved FMLA leave once I had my son.
