i have to change my doctor and place...i moved when i was 3 months pregnant so now its to far...before i could just walk to my apts now i keep missing or rescheduling cuz no money for transportation...i seriously cannot believe i missed this last appt i really thought it fell on a friday...turned out it was that wed..im so bad at remembering thing...i need a day planner

Find one closer that's accepting new patients then call your old one and say due to your recent move you've had to change care providers and to please cancel and upcoming appointments and transfer your medical records to the new doctor you may need to sign a waiver for the records either at your old office or sometimes the new office can fax a request that you sign there
n plus now that im like over 7 months aint it weird that i only seen my dr once cuz every other time she on vaca...which is awesome cuz i love traveling and do things but seriously meet me more than once lol
