Could be yeast. Use gentle soap and make sure that his pits are very dry afterwards. It takes time to dry every crease and fold but it’s worth it to prevent this. As long as it doesn’t bother him leave it alone and just keep the area dry.
I’m not sure that I’d put diaper cream on that. I would just throughout the day keep patting it dry with a cloth. If anything maybe a lotrimin or something for yeast. Call the Dr office and ask to speak with a nurse and they’ll have tips for that 😊
Use some nystatin ointment it’s a yeast build up. You should be able to call your pediatrician and they should be able to send a prescription over to your local pharmacy

Did he have a bath last night? Could just be because it wasn’t dried, and just has some yeast. Little diaper cream will help.
