Do I need to request a form or do I just ring up and add violet onto the claim I already have?
I'm still waiting on her birth certificate (different districts 🙄) so can't do anything yet.

With my son I got it in the bounty pack, with my daughter I had to print it off their site and then fill it in and send it off xx

When ye get her birth certificate ring up and ask to add her it's not a new claim just adding and they ask you for the code at the bottom of the certificate xx

Yeah you don't need to send her birth certificate off when u ring up they just ask for number at bottom of her birth certificate and adds her to your claim x
