This is hippa saying doctors cant share your medical info. You sign these at hospitals and doctors office all the time

This is just like the notice your doctor’s office gives you regarding privacy/confidentiality. All health providers are required to provide you with this notice. You are signing saying you received the notice.

It's a HIPAA notice. All doctors or other care providers are required to inform you of your HIPAA rights, in what instances your information will be used, and what recourse you can legally take if those rights are ever violated. Your signature means you've read the document and understand your right to privacy and the responsibilities of your care provider to protect your information.
