@mikyemommy9216 are you married? I'm pretty sure if you and the father agree on it you can if married, if not then you should be able to give the baby the last name on which you alone choose.

In my state baby can only get mother or fathers last name and they have to be present with an i.d to even give there last name. I would call down to the health department and ask them what they require for last names and such
