You contact the district court. Tell them what you’re trying to do and they will give you a website to go on. You download the form, fill it out sign it, and submit it. They will give you a court date for you to go and explain yourself to a judge. Not sure what this will cost you where you are from but in Texas I was told it’s only $500.

A lot of states require the father to relinquish all rights, if he has any. But contacting a lawyer will give you the best info.
