Filling out paperwork for dds dentist...they're asking for dds social, my social, her dad's social & mine and her dad's drivers license #. Is that normal? Can I leave all those spots blank?
I also imagine if dad ever calls to get info they will want to verify the parents identity. Or if the parents are separated and one of them wants access to medical records? Idk just a guess 🤷🏼♀️
I leave those questions blank. My kids dentist, everything is electronic so I have to sign in on a tablet and it’ll let me go to the next page so it’s really not needed. If it were the tablet I’m sure wouldn’t let me go to the next page without saying error or something.
I usually just put the social of my ex husband because he holds her insurance.
I imagine they are just trying to make sure they get paid? 🤷🏼♀️ I definitely wouldn’t put the kids’ numbers.
@laniejay they look like / seem like the best pediatric dentist close to where we are. Hopefully they either let me leave It blank or have some good reasons! Lol
@k.rose, if they had valid reasons- I’d think about it. I’ve never seen a license # on a form. Socials are usually so they can look up insurance. I just provide the insurance info. I don’t see why they’d need your kids ss. I’d probably find a new dentist 😬

They ask because insurance claim forms ask for that info
