Unnecessary intervention

I went to the high risk OB today for the first trimester screening. These same high risk OB's have seen me during my previous two pregnancies and conducted bi-weekly scans due to IUGR. However, this being baby #4, my regular OB said he didn't think it was necessary this time around unless they noticed the baby was small when they scan at their office every 4 weeks. I appreciate that because it gets to be a pain having so many appointments.

Your frustration is absolutely understandable. I'm sorry you're pregnancies aren't easy. I've been there.

Progesterone is considered safe while breastfeeding.

I'm sorry, Jennifer! I really am not brushing it off at all. If I'm being honest, my post stemmed from just being really disheartened by the appointment. I REALLY just want a low key pregnancy. All of my pregnancies have had one issue after another. I left feeling defeated and like a failure so I'm sure my post didn't come off as intended because I was extremely emotional.

I really appreciate every ones feedback and sharing your experiences with me. It means a lot and makes me less scared about it.

Is progesterone ok to take when breast feeding? I'm still nursing my 1 year old.

Just a side note... A baby with IUGR is not the same as a baby who is perfectly healthy and just small. A history of that alone would be reason enough to consider seeing a high risk OB.

I also haven't been categorized as high risk this time around. I just went to them for the first trimester screening. My regular OB didn't think seeing the high risk regularly was necessary this time around (unless I started having issues) simply because my babies have all been healthy and born at 37 weeks. This is why I was so surprised. I went in for a screening and walked out with like 5 additional things to be concerned with.

I'm not brushing anything off. I said I didn't want to go to the high risk for bi-weekly growth scans when my regular OB has said they can do monthly growth scans at his office when I have my regularly scheduled checkups. I wasn't talking about the progesterone, I have the choice between the shots or the pill insertion. I can do both at home so that is not an inconvenience at all.

I am going back at 16 weeks for the cervix check. I made that appointment yesterday when I was leaving. I was simply stating I have never had an issue with a short or incompetent cervix, so I wasn't sure why he wanted to check it.

I just want to check with my OB about the progesterone. Like I said, I will do it if they both recommend it. Maybe I made my preterm labor issues seem more serious than they actually were. I was sent to L&D once during each pregnancy for it. It was solved and I was sent home in less than 8 hours. I was high risk because I have a septate uterus and my babies are small. Completely healthy, but small.

I really hope I'm not coming off ungrateful or unconcerned. I apologize if that's the way it seems, I would never take having a preemie lightly. I was more asking for people's experience with taking the progesterone. I guess I worded it incorrectly.

Or, in your words, "it gets to be a pain having so many appointments."

It's also "a pain" having a premature or less than healthy baby.

I feel like it's really bad advice to suggest just seeing your regular OB rather than the high risk OB, simply because you don't like his suggestions. Obviously the high risk OB has more specialized training.

While I wouldn't suggest blindly agreeing to everything a high risk OB says, I would carefully consider her advice and ask her questions regarding additional recommended interventions.

It's too bad that it's so inconvenient. Ask some of the preemie mamas here how "inconvenient" a premature infant is. I'm sure every one of them would be willing to have as many extra trips and appointment as necessary for baby's health.

I feel I'm being a bit harsh.

I just feel it's selfish to brush off a specialist's recommendations because they are inconvenient.

Absolutely question everything and know why recommendations are made! But I'm upset at the way you presented this as being inconvenient.

Honestly I would get the shots or the daily progesterone pills and let them check your cervix at 16 weeks!

If its really short than they can place a stitch!

I lost my first child due to pre term labour you should be happy that your high riak ob is being so pre cautious!

I know alot of woman that went into pre term labour and with their next pregnancys the dr wouldnt monitor their cervix and they lost another child!

The progesterone will just help prevent pre term labour and help keep a long cervix!

I had the progesterone shots daily until I was 13 weeks. They aren't that bad, there are no downsides for them whatsoever (except hot flashes), and we were able to do them at home. Yes they are uncomfortable but to keep baby in as long as possible I would do them in a heartbeat!

Do your regular ob and high risk doctor communicate at all? Hopefully you can figure out what will work best for you. I could understand where you are coming from but at the same time it seems like going into labor so early wouldn't be a risk I would want to take if there is a shot I could get to prevent it. I've never been in that situation though so I can't speak from experience.

Thank ladies! I honestly don't think my regular OB will agree with him but we will see. I will talk to him about it during my next appointment. With the first, they just gave me a shot of terbutaline, second just IV fluids. All my babies were 37 weekers, just tiny little things.

Of course I will do whatever is recommended. My regular OB just knows me and my history better. The high risks rotate in and out and sometimes I just feel they want me to come in for every single thing.

For example, my regular OB said they can do growth scans in their office starting at 24 weeks to monitor measurement. If the baby slows down, they can send me for closer monitoring at the high risk. When I told the high risk that he said he didn't understand the point and said I should just come to them the entire time.

I think sometimes the preterm labor gets worse/earlier with subsequent pregnancies.

I would definitely ask your doctor to explain WHY he's recommending the shots. I hate not knowing why I'm being asked to do something.

You explained perfectly why it seems prudent to take the progesterone. You have a clear history of preterm issues. I took weekly shots starting at 16 weeks. I had to drive 2 and a half hours round trip, every week for ten weeks for them. It was worth it, I carried my son to 37 weeks (longest I've carried). I'd recommend taking them, it's a hassle but your lo health is all that should matter.

Doctors have a degree for a reason. My sil had her first early. Got the progesterone shots with the second he was born term. Her third was early because she stopped getting the shots two weeks later he was born at 32 weeks.....better to be safe than sorry imo.

I agree with the pp. It sounds like they are trying to avoid preterm labor all together since you have had two incidents with it even though they were able to stop it.

To me it sounds like it would be better to get them and hopefully avoid having to stop preterm labor. I would make a list of your concerns and talk to them. Goodluck!

I am so stressed right now!!!

I am so stressed right now!!! My OB doesn’t want to take me anymore because I am too “high risk” so I got a new one they don’t want to see me either. They referred me to a group one hour way from my house. But there is only one high risk dr and he doesn’t know if he could see me. I feel like I should not see any at this point.
