Don't forget you have to have the counter signatories passport number as well, I think it has to go on the photo and on the form too xx

I just did charlies everything was on the form again along the lines of I certify this is a true likeness of (name)

Name of countersign

They sign it

Date

They may need to add dob as well

I have just sent mine off and had it checked and they said it was absolutely fine. The photo is signed the child's name so it says that it's the true likeness of.....(child's name) the form is about the parent so there countersigning the parent on form child on photo. Xx

You put child's name on back of pik and ur name on form. Just got kids passport done and this was bit that confused me so much but luckily took 2wks for passport to b ready

It says in the leaflet and on the form I believe. It's something along the lines that I certify that this is to the true likeness of...... Something like that it's written exactly how it needs to be said tho xx
