I believe if it’s projectile, abnormal output (not enough poo or wet diapers), there’s blood, their behavior is off those are causes for concern. I’m not 100% sure so don’t quote me. If you’re worried give he pediatrician a call. There’s also a 24 hour nurse line I found super helpful!
