I heard that adding cereal could be a choking hazard

My son had a terrible problem with spitting up, eventually I just mixed a little bit of rice cereal with his formula in the bottle. Made the hole in the nipple a tad bigger because the cereal needed to fit through it. You don't add too much though just enough to notice a difference... The formula alone might just be too thin for him.
