So this hospital I'm at wants a birth plan. Did you ladies use a template online or just make one yourself?

I never ended up even giving the hospital my birth plan, but I had made one using a template online

I use one online. Different ones ask different stuff. Like the app 'babycenter' has a pretty decent one.
