No that's not standard. It's different in every state. Sometimes if you get a drs note they'll cover it.

Yes if they dont cover it speak to your doctor they will write a prescription and insurance will send you the formula you need

That's weird but you can just call the doctor and see if they can give you a prescription for it!

Most have you get your doctor to approve of it! I know Massachusetts and New Hampshire had me and my friend get a form filled out by the pediatrician
