As long as he doesn't start to puke or act different no, he does look like he hit it hard but if starts throwing up yes take him to a Er he could have a concussion

Lots of snuggles and just keep and eye on him! Poor baby

:( if it helps any, my kid has a permanent knot on his forehead from where he always hits his head.
