How can you have a baby boy and he is in preemie clothes, but the thing is that he was born 41 weeks?

My 40 weeker was small enough for preemie clothes and diapers for the first month or so. Some people have small babies.

My 37 weeker still can wear preemie clothes and diapers and she's 7 pounds now. She's still tiny.

So it's not just the drugs that my sister is on? Good to know that. I don't mean to be rude if anybody took it that why. Just curious it could be other things other than drugs.
