Ladies! Help! I went to the Disability office and they told me to fill out this green packet and that my doctor has to fill it out too and then I mail it out? Is that how you ladies did it?? The guy at the office was kind of a dick and I didn't want to deal with his nonsense. 😠 I'm due in February so I'm not sure if it's still too early to be filling it out. I just need some help. Please, thank you, and I really appreciate you ladies. 🙂

I actually had my insu fill it out I had the doctor do his part then turned it in to my insu and they did the rest

@ingridm89 yes thats how it works you and your dr fills it out but you will have to ask your dr when they will do it for you, i suggest you drop it off at the drs ASAP because my DR took 2 weeks to fill it out for me, some places you cant apply for pregnancy disability until 34-36 weeks and it can take 4 weeks for them to process the claim

i applied for it at 35 weeks and put the date i would be going on disability the day i was 36 weeks, my son came the day after i went on leave and it took 3 weeks for them to approve it and mail me the card with my funds on it. @ingridm89
