human resources.. they'll have you fill out a FMLA form (The Family & Medical Leave Act ) to make sure you qualify and then go from there

There aren't any maternity laws in Arizona, it is only FMLA and you can look up online if you qualify. If your employer does not provide any other type of leave then you need to get the FMLA paperwork from HR and give it to your doctor to fill out.
