I get this awful pain after I pump in my right breast. It feels like it goes all the way to my back…

I get this awful pain after I pump in my right breast. It feels like it goes all the way to my back too. It's not a clogged milk duct, I've had that before and this feels different. I can't tell if it's from completely emptying my breast or not but it's an awful pain and I can't do anything until it goes away. Can someone please help me with figuring out what it could be?
