you can print form off and send it yourself hun.. i did mine was back within 15 working days xx

You have to fill in the forms from the job centre and send off 13 weeks worth or payslips, original matb1 form, and the sheets to why your boss can't pay you smp. It took 3-4 weeks for mine to be sorted. They also backdate your money too, so even if your just claiming now or even after baby is born then on the forms but the date you started maternity and they back date it (: hope that helps xx

There is a 66 week test period that is calculated from due date, 25th week of pregnancy etc and you can choose the 13 weeks (or 4 months depends how you're paid) which you earned the most in between the given dates and those are the payslips you send along with the other evidence stacey804 mentioned. I'm waiting to hear back about my maternity allowance claim
