Im going crazy & getting frustrated trying to find the forms for short term disability. All I find are list of qualifications. If anyone can help I would really appreciate it.

If you do it online, they give you a reference number for your doctor and they can log on and sign their portion of it online. The application gets processed way faster if you do it all online @twinsmama16
thank you ladies. My doctor took me outta work sooner then I was planning & told me to bring in the paperwork so they could sign off aa high risk. @sarahb518 &@rachel8792
Its true, you cant file until the week of. Mine didnt go thru until the day of, but my co worker had it done the week of
You can only apply like a week before you plan to take your leave. I waited until the day after my last day to file. It will reject the application if you try to file it too early. It's a pain, but you can't do anything until you are ready to take your leave. @twinsmama16
thank you @sarahb518 I started an application there but they ask date benefits began & Im barely applying & it wont let me go further without a valid date.
You apply online at edd.ca.gov Just palsy for disability. In the application you can put for maternity leave and your estimated date back to work.
Yesterday I went out with my boyfriend went to watch a movie, me and him don't live together yet. but point is yesterday when he was dropping me off his like sleep over at my house witch the mom and dad lives. I said no,
his response was why is everyone deciding or making choices for you and their going to do the same with the baby, when I know that's not true. what he thinks is that my mom tells me not to sleep over. So he left all mad. and I felt in my heart that he went on and cheated on me...

Short Luteal Phase?
I've just started temping because I believe that the reason I haven't had a BFP yet, after 20 months, is because I believe I may have a Luteal Phase Defect.
