Most states require photo ID social security card and marriage license, some require birth certificate. Call your local SS office for what you need in. Your state and county

ohio requires bc marriage license ss card and Id to change ur last name should b the same everywhere cause they r all linked together

@tayhuth, that is probably something you will have to talk to your SS office about because 90% of the time you need it

Yes I know from experience they won't do anything for you unless you have everything they need. It's frustrating. You can always call and ask them before going.

@tayhuth, some places might but it's not likely. Just call them tomorrow and ask, it's much easier that way

no usually they won't unless it's a notarized copy

I needed a certified copy of my marriage license, my drivers license, my social security card, and my birth certificate when I changed my name in February. It doesn't matter what state you're in since social security is a federal entity.
