Anyone have gateway insurance and have done this and received a car seat, stroller, or pack n play??

I have something like Gateway and all my doctor had to do was fill out this paper, I got the pack and play and am keeping it at my grandmas since she unexpectedly bought me one. The pack and play is just like the ones that have the bassinet and changing table in them only they don't have the bassinet.

I have gateway, where do I get this form tho?

How do you get this?

@rainbowbaby2015, ah! Okay thanks for the heads up about how it is, do they give a certain color/design? I'm also getting the pack n play from there since I already have the stroller and car seat.

No they use unisex like mine is brown with animals on it (giraffes, lions, etc)

I'm going to my grandma's later I can comment a picture of it then

@lovelybabybump27, All you have to do is call gateway and tell them you're interested in the "MOM Matters" program and they will mail it to you :)

@lovelybabybump27, actually I have the mother to the moms matter program here you go you can call them :) 1800-642-3550 option #2 they're open Monday through Friday 8am to 5pm
