QUESTION: I called one of the local WIC offices that's a part of my hospital I go to and will be using when I deliver to ask what they'd need from me if I wanted to receive wic. I was told that because I'm not on a state insurance program and have insurance through my father's job, I need to give them pay stubs (obviously) to prove that I meet the financial requirement, and I need some form from my OB and then I can go down to the WIC office and set up an appointment. Did anyone else need a form from their doctor? And if so is that something you needed to get while at an appointment or could you just pick it up if you call? My dumb self forgot to tell my doctor about it at my last appointment on Friday.

What type of form do you mean? I had to bring proof of pregnancy which could be an ultrasound, or in my case I went to this place called Birth Choice, took a pee test, and they gave me official proof of pregnancy.

@babyboy582016, okay thanks! It's weird because the hospital and every doctor and office affiliated with them have all my medical files electronically with no need to even ask me to fill out anything, and can see everything going on from doctors visits and appointments, to test results, and everything and would clearly be able to see from that I'm not lying. I'll call my OB still just in case but I just found the whole doctors form a bit odd.

@babymay2016, the lady on the phone literally just told me I had to ask my doctor for a form and that they'd know when I asked what to give me. I have 3 sets of ultrasounds so far, they can electronically pull up all my health records which all state that I'm pregnant, and my confirmation appointment summary where it shows that as well because all my doctors are affiliated with the hospital I will be using. I unfortunately can't go up there today because I have to work but I will for sure be taking those items that I have up to them tomorrow if my doctor says that my ultrasounds and everything should be sufficient.

besides being on state insurance all I needed to do was take a form that WIC gave me and have my doctor fill it out. basically stating my due date. an ultrasound was not sufficient enough as proof of pregnancy.

@riverbordengovoni, they told me that because I'm not on state insurance I have to get some form from my doctor and bring in pay stubs. The lady I talked to on the phone wasn't the kindest or most helpful person to make matters worse so I'm going to have to call my doctor to ask about it later because she didn't even say that I could get the form through WIC and that I just had to go to my doctor

your doctor will most likely print out a proof of pregnancy letter basically stating you are a patient and what your due date is. but when I had my appointment they gave me a blue form for my doctor to fill out and bring back. but like I said I'm on state insurance so that might be why they had me do it that way.

I also live in Massachusetts, so WIC might be different in MA than it is in CT.

The form is generic. You can either pick it up at the WIC office, see if your doctor have copies or you can print it online from your state WIC website. Your doctor needs to fill it out, it's basic information including due date, lbs, blood results and the doctors stamp @jc100495

All you have to show is proof of pregnancy. It's the script that they give you in the beginning
