your manager still should've pulled you aside to inform you. they try to not tell you so that you think the only way is to quit. my manager was shocked that I asked for the papers after a co worker told me. I also didn't know that it could take up to a few weeks to fill out the paper work at the ob office, so when you take the papers there, explain what happened so they can try to hurry up, and if your not at work rite now, call the manager to get the maternity leave papers ready or the absence of leave papers.
NP. its not your fault. its the manager because she should have talked to you about turning in papers. I worked in retail(marshalls) while I was pregnant with my son and they tried to do that, and the some co workers told me to call H&R(headquarters of the company) and then my manager gave me the papers and I got 12 weeks off.
