my doc had given me a paper and I took it to my work so they can file it out and then send it out

Ohh okay! It's cause my job is only seasonal till April 15th so I don't think they would send it out to be on maternity leave since there only really open for about two more months :/ @1130nnne

my doctors office has you do it online then you give them the receipt number and they fill out their part on the computer so no paper needed! just ask cause all doctors offices aint up to date! do it soon cause it takes awhile I had my baby and im going back to work in a week or so and I still haven't received mine. When they put me on bed rest I had a doctor note that I gave to work and then I just filed online for disability they then contacted my Dr and approved it all and we're able to just look up how much I make and hours of my job though my ssn and your works address (: hope it helps. just warning you it delays paying you for over 2 weeks so the first 7 days of leave yoy don't get paid for then yoy have to wait to get your debit card and finally get paid.
