1/1500 cases fail. But, she figured out, it's because they had sex to soon after! They can "fail" when the remaining live sperm downstream from the vasectomy site aren't completely “flushed” out of the system. Typically, it takes 20-25 ejaculates (or 3 months) for the live, downstream sperm to be completely eliminated from the tubes after a vasectomy. So, use a condom for the first 3 months, just to be safe! Haha. She's quite surprised.
