talk to your doctor if it's disability due to the pregnancy, but I'd definitely talk to someone else at your job they should have what you need because if they don't I think that's bull crap

well my doc is doing it but he needs my job to send the forms which they don't have like they have no clue where to get it I'm like seriously my doctor need that for ASAP @aphrodite2014

@aldensmommy, is it through FMLA? and if your job has a corporate office or something I'd definitely talk to them!
