@kimberlyyyy, no i thought that last time i changed his diaper size and switched creams . and i use pampers i dont use other kinds cause they use to give him a rash so im not too sure now what's wrong

Yes it comes and goes for my son. It's pretty common for boys to have problems like that occasionally. Maybe get a warm damp washcloth, take his diaper off of him and put the washcloth on his privates and just let him chill like that for a while. I bet that would help!

I ask snout breastfeeding because I thought maybe he would be uncomfortable with something you ate. Anyway.as @queendavis is getting at, the ER probably don't do much. You just have to try making him as comfy as you can. You may need to switch diaper creams and have more naked time. Could you be putting him in too small of a diaper? There's so many things that could cause him to be uncomfortable.

Also was he red down there? The ER won't do anything for that btw so it would be better to stay home and not waste your time and money.

You don't need to take him to the ER. Was it his testicles or his penis that you touched and saw that it hurt him? For some reason my sons testicles get really dry and red sometimes and I can tell it bothers him, so I usually make sure to put moisturizing lotion on them after his bath and I use diaper cream on him at almost every diaper change. His privates could just be irritated. If he's awake, a warm bath could help him feel better and then I would just put some diaper cream on him and see if that helps. Sometimes those diapers can rub their privates so much that it can irritate and rash them up. Poor baby hope he feels better.
