a&d is good but Triple Paste works really great and lasts longer. It helps form a barrier so acid and yeast never touch the skin. My boys were allergic to Desitin and we ended up getting a prescription of Nystatin from the pediatrician. With this baby I plan on using the Triple Paste which is so much like the prescription. it could also be the diapers you're using

try a&d ointment, just strictly a&d. we use that on my daughter, and we do it every time we change her, and wipe her down each time, because of the acid in pee, but that usually works for us.
