ya you just have to sign a ralease form and they will send it by fax. It's 2015, there's no way that they cant get it done instead of going to your old doc personally.

in order to be seen by a new doctor they need that record. they need to know about your pregnancy so far and if have any special care for your pregnancy. just call your doctor and tell them to FAX it to your new doctor. there should be 0 problems with doing so
