okay ladies I need help because vital statistics won't help me !!!!
so I sended my paper work to get my baby birth certificate and I got everything back because the stupid hospital spelled my baby name wrong and didn't put my baby daddy on the birth certificate!!! like seriously everything they gave me with paper work his name was right and baby daddy name was on every paper work!!
so now idk what to do!! idk how to change his name and how to put my baby daddy on the birth certificate!!! please if anyone can tell me how to do it I would really appreciate!!! thank you!!!

I know you send it in the mail and I had to pick it up at the court house but I paid 20 when I added my son's father on the birth certificate. I would double check with the court house.
thank you !! so j just fill the out and send it by mail and they will send me the birth certificate? without paying anything? cuz I already oayed 20 dollars but I didn't get the back.
you get a afadavid (sorry don't know how to spell it) and have the dad sign it and he will be put on the birth certificate. you can get it at the court house.

call your local social security office they're tell you everything you need to know.
