Thoughts: If a nurse passes out a med and gave the wrong dose by accident and it killed the patient, Do you think the nurse should be criminally charged?
What if she told the higher ups at the hospital and during investigation they tried to cover everything up but once everything came out they put it all on the nurse?
Should everyone be charged with the same criminal charge?
Should they not be charged but be held accountable and not able to practice?
Mistakes happen so nobody should be in trouble ?

Yes. She should be charged with manslaughter. That’s what anyone else would be charged with for accidentally killing someone.

This is a breakdown of so many issues. There should not be lethal doses side by side on a cart. The system usually has to dispense the really tricky meds and another nurse has to sign off on it also before it is given. If the nurse wasn't paying attention and gave the same meds twice, they're probably going to just get discipline or the process is going to be changed. They are so short staffed on nurses. And don't get me started on the time an on call doctor was wasted and trying to prescribe a lethal does over the phone and the nurses had to walk them through the thought process to not kill someone.
There's been talk of having meds dispensed automatically by a machine and having trained nurses aides give them, which is a bad idea. Knowing when to hold a med is as important as knowing when to give it. Anything to make health care more profitable in Murica.

That override shouldn't be possible or only in extreme circumstances. There's a lot of wrong going on here. That hospital knew what they were doing allowing that override constantly. Understaffing BS.

if I was that judge, I would want a report of the overrides on that unit for the quarter. It's probably insane. Yes, the nurse is wrong, but the hospital environment is really wrong also.

There are sign offs for this so everyone would be accountable in these situations. I would believe but I am not sure. We use this system at our hospital.
