I never got my sons ssn and now i have to send documents to get it. I have to send 2 documents to proof his identity. Im sending his birth certificate and what other document can i send? I tried calling but no answer

There should be a list on the SS website. The same thing happened with my son when he was born… I think we sent in his birth certificate and also the hospital record of his birth. I had to pick up an original from the hospital and have it notarized.

I called his pedi and im getting his immunization record and doctors visits. How would i know if i have to get those papers notarized?

@mommyof.1, Just have them stamp it or sign it in colored ink. Don’t let them do black. The hospital used a black stamp and the idiots at social security thought it was a photocopy instead of the original. You could clearly see it was a stamp and had bled through to the back of the paper. I had to go get a whole new copy with a colored stamp. It was a huge pain in the ass. I’m pretty sure there are directions on the ss website with the criteria for each eligible item.

I think you should schedule an in person visit with your social security office and go from there. To my knowledge you can only apply for adult social security cards via mail.
