I’m about to pregnant cry I need some help with a form. So, long story short when I had my daughter I put down my ex on her birth certificate. He’s not the dad and I have dna showing he’s not the dad. So I want to remove his name off the birth certificate and change her last name. They gave me a form “petition for name change” . An I filled some of it out but I don’t know how to fill the rest out and they won’t help me. They said I have to pay for an attorney. It’s just one paper. An I just need a little help filling it out. I don’t wanna pay thousands of dollars for two questions. Anybody been through this mess ???

If you're removing him and the question is asking who you're removing I'd put his name. You may want to see if you could get a pro bono attorney just to avoid anything crazy happening
I doubt it would be thousands. Most attorneys charge a couple hundred an hour. You could hire one to help you with one document.
Where I’m removing my ex off the birth certificate I don’t know if i put his name on the form or not. Or if I have to put his name down because he’s technically listed as the father. I filled out all the basics. I just don’t know what to put for a couple.

@momofeach, You will definitely need an attorney or family lawyer to remove your ex off the birth certificate. You can’t just do it with a form your only “applying” to. Although you shouldn’t need to pay for help to fill out the form that’s just silly! BUT often they aren’t legally allowed to help you fill it out that much I do know in their eyes it counts as giving legal advice.
Even with DNA the way birth records work now is if the man signs it he’s signing to being the biological father OR taking responsibility for the child, so whether he shares DNA or not isn’t always relative. My sister just went through this and it was quite a detailed legal process.
You can also reach out to your local legal aid society. Try to look them up on google!
