he’s breastfed and sometimes gets formula

Okay well then it's most likely the formula that's making him constipated. It happens a lot. You can try, bicycling his legs or rubbing Vaseline around his hole to help him push it out. I've had to use those trick's on my niece's and my daughter. You can also call you doctors office and they can give you advice too.
