You’d have to request a new one through social security. You only get 5 replacements- even after name changes. I suggest when you get new ones not letting a 5 yr old have access.

Call your local Social Security office. They will tell you exactly what you need.

You'll have to go to the social security office. Make sure you have a birth certificate and they are gonna ask A TON of questions. One that they asked me was all my addresses for the last 10 year's and about family members.
