Don't know what to do

My beautiful little boy is 4 weeks on Wednesday. During the day the only time he is quiet is when I'm holding him. I put him to sleep, put him down and within 20mins-1hr he is screaming. Same at night. Will only sleep an hr at a time. 2 IF I'm lucky.
