I'm actually going through something similar. We started supplementing small amounts of formula, 1-2oz after every feeding. Also, my oldest had a tongue tie and it looks like this little guy may have a posterior one, which is why he's not getting much from breastfeeding.
My lactation consultant was able to diagnose this just by watching him feed and weighing before and after a usual feed.
We have our 2 week follow up on Thursday and fingers crossed he has gained. Next step for us is to go to the oral surgeons to have him evaluated.!

I supplement if needed. It's only 1-2 oz if I didn't pump enough for the feeding. He means don't give up and just supplement if needed. If my baby eats 3-4 oz of breast milk I don't need to supplement unless she is still showing hunger cues. You are doing a great job and don't get discouraged. He is gaining but slowly. Just use the formula as needed.

How do I know if I need to supplement any formula? I'm not sure how much he gets from the breast, but I let him feed until he unlatches (I try to listen to swallows too) and then I switch him to the other side until he unlatches from that one but then he still seems hungry and still sucks on his hands and roots. So then I would give him the breast milk bottle and he would drink about 1-2 oz before like passing out lol.

Are you able to go to a lactation consultant for a feeding weigh check? It helped me a lot. They weighed him before and right after a feeding to determine how much he was getting.

I've seen 2 lactation consultants but neither suggested weighted feed. If he hasn't gained by Monday I'll bring it up to his pediatrician

You could probably do that at home too on your own if you have a scale and then just give the pediatrician the results and ask what they think. I would think that if he still seems hungry after feeds then give some formula or call the pediatrician back and ask if there's a specific time of day they want you to give the formula and how much. I hope you figure it out! I know breastfeeding can be really stressful. I had a lot of trouble with my son and this time around I'm determined to make it work and it's really proving to be difficult, but I'm pushin through! Good luck mama. Even if you have to mostly give formula but you give breastmilk a couple times a day, the baby will still benefit from that!

That's true Bairdclan. That's awesome that you're pushing though! That's definitely what my goal is. I think I will realize too that whatever I need to do will be clear to me.
I'll try to weigh him on my scale at home but I know it won't be as accurate as a baby scale would be. But it's worth a shot!!
