Is he having some difficulty accepting his increasing age? I can relate. I didn't want a 35th, but it happened. Even without the cake, I still aged. Maybe he doesn't want to grow up. Adulting is hard, and every lap around the sun brings you THAT much closer. He sounds smart.
Maybe ask if he wants to go out somewhere. Definitely *not* to celebrate his birthday but to just have a fun day that just so happens to be on or near his birth date?
Did something happen at someone else's birthday that has affected him? It seems unusual to me because my DD is obsessed with birthdays at age 2 and everyday it's either hers, mine, DH, or the dogs birthday. He sounds articulate enough, have you asked him why he feels that way?
