All you should need to right is you'd like permission for your (insert kind of dog even how old) to stay at your address. They need everything in writing that's doesn't have to be anything too fancy than they'll ask the owners and get back to you

I'd add that the dog will be kept outside at all times and that the yard will be left in the condition you moved in. Any damage to the yard will be paid for and rectified by you before moving out should any occur. I would also specify breed and size and that your dog will attend obedience training. I would leave out the fact that this dog is already yours but add in that you will be getting a dog not a puppy. I think that should cover it.

We were approved for lease with 1x female German shepherd cross. After 4 weeks we approached the real estate about a second dog as our girl had been diagnosed as self harming and depressed (I know right high maintenance) and our options were to medicate or get her a friend. Out request in writing basically stated that we are seeking approval to keep a second dog on premises, stated the breeds we were considering (german shepherd, boxer or mastiff) and that as tenants we would ensure any and all damage caused by said pets being kept on property would be rectified before moving (standard disclosure on all lease agreements containing pets) and that if any complaints were received from council or neighbour's that the dogs would be rehomed (this is not standard) I reworded the last disclosure to state that we would re home them only after all possible measures were taken to rectify complaints and that a complaint was only considered as such if recognized by council) and wallah dog was accepted.

Just keep it short and sweet. They may want a photo of the dog too. Don't over think it, they just need it on paper as part of the paper work trail. :)
