How well do you know this person? If it was my sister or best friend I would say something. If it was some random acquaintance I certainly wouldn't, personally. I don't think it's your place to say anything unless you know them well. They probably have other people in their life who will see the picture and mention something to them. Chances are they know it is not recommended though. It seems to be pretty common knowledge in my opinion.

I don't know if you heard about the mom who just lost her 7 mo old due to an unsafe sleep environment. A blanket suffocated him in his crib. So incredibly sad but the mother shared her story to hopefully save another child's life. Personally, after following her story, I would say something.... just in case they don't know and you could be saving their child's life.
If you don't know them too well then why be bothered about what they think, fact is your giving them advice that could save their child. If they don't like it then at least you tried. I'd rather lose a 'friend' than keep safety information to myself. They may respond defensively but At least they now have the knowledge

So I am close with a girl who knows them better than I do. I just told her it may seem better coming from her. She said she's already told them. They said he's lifting his head so he's fine. She's expecting her first and she told me she gets so nervous seeing these things but she told them and they don't care. :(
