I think you can fill it out they just might gave to wait to process it. I think it depends on your insurance. I have UHC and they won't process the request until a month before.
I did byram healthcare online. They did all the leg work with my insurance i just filled out a form and picked out my pump. I chose the spectra s2. They sent it a whole month early. Only thing id recommend is making sure what size flanges tou actually need. My set came with the standard 24mm i need a 30mm which i didnt know til after birth so im renting one till my new ones come in.
