I have been using all free and clear everything for him I will call his main dr tomorrow I took him to a dr office or urgent care

I would get a second opinion from a dermatologist. I once had a PCP do a pathology test on eczema.

But it does look like eczema. I’d start with changing laundry to a free and clear and stop using soaps that have fragrance in them. Even shampoo and conditioners can cause irritation.
