Lady’s I need some help filling these out for a breastpump I know I feel dumb lol but what do these 3 things mean are these suppose to be phone numbers?? Or my ID INSURANCE CARD????😫

There should be a number on the back of your insurance card for services put that in the last. Second one is the ID or member #. 1st should be your policy number that is on your card.
