I think that's a bit too young for most OTC syrups. For a cough/runny nose run a humidifier or sit in the steamy shower. You can also cut an onion in half and put it in a bowl near where he sleeps.
I brought my little guy in his bouncy seat into the bathroom when I would shower to help relieve his stuffed up or runny nose. I don't think there is a cold medicine for babies younger than 2... could be wrong but that's what I did and a fever reducer to help him sleep and lots of cuddles. Plus a humidifier near where he slept.
None don’t give your baby any cold medicine take him to the doctor ASAP if he’s sick and they will prescribe the medicine that he needs and will be able to take
4month old is way too young for any medicine that a doctor doesn’t approve first

None. Warm bath (99-100 degrees) and a humidifier.
