If there's no fever or anything alarming besides fussy I wouldn't take him in. Just wants extra cuddles or could even be teething. I have a baby in my daycare had 2 teeth at 4 months.

No sounds like the four month sleep regression. look it up and if it sounds like that then I would just go with that
