To those who have applied for FMLA? What form did you fill out? I'm having trouble understanding…

Make sure to call your local sdi office...but for now you sign the disability one first..if c section u have 2 extra weeks i think non paid...anyway then the bonding time...you can extend your bonding but once again nonpaid....

Oh your doc has to sign also so make sure that you save code

Xochilt, you have to do a green form first. Your doctor has to fill it out as well as you. That form gets sent to Sacramento. You're going to get a letter about your wages and stuff and that's when you'll start to get paid. When your disability is close to ending they'll send you a paper for bonding time. :)
