Why Does My Daughter Keep Peeing Through Her Diaper?

The last three times we've been out my daughter has peed through her pants and diaper. We changed her before we leave and while we're out. First time we were at the bank and she was sitting on her dads lap well ten minutes of having a conversation with a banker my husband says "Our daughter just peed all over my pants." The second time we went to our insurance, we walk inside and are getting ready to sit down when she goes pee all over his arm. Then today before we left I changed her diaper. We go out to eat, we haven't even been gone an hour and as we are leaving my husband notices that she pooped through her pants. The joy of being a parent.
